In appreciation for completing this online survey, participants will be entered into drawings for one of ten $15 Amazon.com gift certificates and a $100 Amazon.com gift certificate. The link to the official rules may be found on the first page of the survey.
Survey participants will also receive an advance copy of a report summarizing the findings before the results are published.
The survey should take about five (5) minutes to compete.
